Address Changes
If you move (or change
employment) and can no longer be reached at the address listed on your
rubber stamp seal, you must file a change of address. Take these
steps:
- Order a new rubber
stamp seal with your new address and your current expiration date.
- Write a letter
requesting that your address be changed, and place an imprint of
your new seal at the bottom of the letter.
- Mail that request
to the Secretary of State along with
the fee of two dollars.
Name Changes
If you change your
legal name through marriage or by court action, you must file a name change.
- Order a new rubber
stamp seal with your new name and your current expiration date.
- Write a letter
requesting that your name be changed, and place an imprint of your
new seal at the bottom of the letter.
- Mail that request
to the Secretary of State along with
the fee of two dollars.
- While you are waiting for the change to be processed, sign
notarizations using your new name, and write under your signature
"Commissioned as" and your old name. Continue to use
your old stamp until the new one is approved, then destroy the old
stamp.
Name & Address Changes
You may change both name and address with a single filing and a
total fee of four dollars. Follow the steps given above.
Resignation
If you leave West
Virginia, or otherwise decide to resign your commission, you must resign
in writing and destroy your official seal. West Virginia Code
requires that you "send forthwith by certified mail or
deliver to the secretary of state a letter of resignation and all papers and copies
relating to his notarial acts." [WV Code
§29C-4-402]
