Forms & Publications

Write our Notary section at notary@wvsos.com to request forms and publications, or call (304) 558-6000.

Questions About Your Pending Commission?

Remember, it takes about three weeks for a commission to be issued after the completed application and payment are received.  If you have not received your commission letter within two weeks after mailing your application, please contact the Notary section at notary@wvsos.com or call (304) 558-6000.  Unfortunately, some letters are lost in the mail, so let us know if you are concerned that we have not received your application.

Data and Lists of Notaries

Only the names, addresses, residence counties and commission dates of notaries are public information.  Other information in the application or the notary's record is not subject to release.

The notary directory information is currently maintained in a legacy database.  Lists and data files can be generated for:

  • All notaries whose commissions are currently active.
  • All active notaries in the same city or town.

Additional options may be available in the future.  For use by the purchaser only, the charge is $25 for the programming charges, plus the cost of the list in paper format ($1.00 for the first page and $.50 for each additional page).

Data created and maintained by the Secretary of State may not be resold without a contract for resale, in which the reseller reimburses the Secretary of State annually for each instance of resale.
